How does it start?

It all starts with you. Whether you’ve been thinking about having a physiotherapy service as part of your PCN for a while, or it’s a recently formed idea that has been discusses with other clinical directors, we offer a unique approach that is tailored to the PCNs requirements.

Presenting & Planning

  • We will come to you, discuss your needs and present how Sano’s FCP model can work for you. We will provide information packs, tangible outcomes and examples so all your questions are answered.
  • We will assign you a Customer Success Manager, who will be your single point of access and will work with you to understand your requirements. They will then ensure the service meets (and exceeds) your needs while always advocating for you to ensure you are at the centre of all decision making.
  • Once a contract has been established, we will work with you to plan the rota based on the hours required and the clinical space within the PCN. Sano manage all clinical supervision for our FCP’s directly and time is build into the rota for this. Once this is agreed we then start the service!

 

 

Induction & Communication

  • Our FCPs will have a brief induction with each practice they are set to attend and are encouraged to join any relevant training/ meetings that the practices hold.
  • As soon as the service starts we record data regarding utilisation and clinical outcomes, this is then complained into a monthly report that is provided back to your PCN. We review this data with your PCN to ensure the service is being used to its full advantage and take action on any opportunities for improvement. You will be given a point of contact at Sano, any questions or queries you have about your FCPs or any aspects of the service will be answered promptly.
